I've spent nearly twenty years in industrial automation — the early years commissioning control systems on plant floors, from oil and gas to packaging, where one wrong line of code stops the line, and is traced back to its source within the hour; the later years as a partner leading the engineering teams that build those systems for energy and data-center clients. What that career taught me is that systems almost never fail from one catastrophic mistake. They fail from small decisions made without a view of the whole — and from temporary fixes that quietly become the way things are done. Companies fail the same way — they just decline quietly, in minutes and ledgers, visible only in quarterly reports and the headcount purges that follow. We should be honest about who this is not for.
